No more than 2 prescription refills will be allowed between office visits.
A controlled substance has not been requested; these medications require an appointment. For example narcotics, codeine, etc.
An antibiotic is not requested
The medication was originally prescribed by a Salveo Family Medicine Clinic Physician
The pharmacy of choice with phone number is provided.
The $50.00 Payment for prescription refill has been obtained in full prior to prescription being filled.
The medication is on the approved list of Allowed Prescription Refill Medications and is approved at the discretion of a Salveo Family Medicine Clinic Physician.
A Follow up appointment has been arranged.
